Temporary Leave of Absences
To apply for a Leave of Absence, fill out this form to log your request in our system. You must use your HCPSS account to log in.
Family and Medical Leave (FLMA) Disclosure
Eligible employees may take Family and Medical Leave (FMLA) for absences greater than three days for a qualifying condition. If eligible, FMLA will be applied to all qualifying temporary leaves of absence.
Qualifying Conditions
- Personal Illness – for a serious health condition that prevents the employee from performing the essential functions of the employee’s job.
- Maternity/Paternity/Adoption – for the birth or placement of a child for adoption or foster care.
- Family Member Illness – for a need to care for the employee’s spouse, child, or parent with a serious health condition.
- Military – Qualifying exigencies arising out of the fact that the employee’s spouse, son, daughter, or parent is on active duty or call to active duty status as a member of the National Guard or Reserves in support of a contingency operation.

Personal Illness
Employees who have been/will be absent for more than 10 consecutive days need to apply for a leave of absence with the Office of Human Resources.

Military
Employees who have been/will be absent for more than 5 consecutive days need to apply for a leave of absence with the Office of Human Resources. Eligible HCPSS employees are afforded 15 days of paid military leave on an annual basis.
