In order to maximize the learning experience and provide continuity between home and school, please return this form with the following:
  • Two copies of all necessary texts, unless the student already has his or her own. A teacher's edition would be greatly appreciated.
  • List of units/chapters the student would be expected to complete.
  • Two copies of handouts/worksheets the student would be expected to complete and an estimated time frame.
  • Any assessment instruments you would like us to use.

We ask that this be completed within 48 hours to allow students to begin home teaching immediately. Please keep in mind, only classes for which we receive instructional materials can be taught.

At the end of each marking period and /or at the completion of home and hospital teaching, grades will be sent to the Registrar/Guidance Office. If the student returns to school prior to the end of the marking period or academic year, these grades should be averaged proportionally with the grades earned while the student was in school.
